By checking this box, I hereby authorize Lifetime Moving & Storage to charge the above-mentioned figures to my credit card for moving services. This “Signature on file” becomes his/her agreement to pay all charges as indicated above. I guarantee and warrant that I am the legal card holder for this credit card and that I am legally authorized to enter into this credit card agreement with Lifetime Moving & Storage. Delivery balance is due via cash or postal money order only, unless otherwise approved by a manager and in writing. A 3.5% credit/debit processing fee will be added to your remaining balance. Storage Recurring Billing

I understand that this authorization will remain in effect until delivery of my items, and I agree to notify Lifetime Moving & Storage in writing of any changes in my credit card information or termination of this authorization at least 7 business days prior to the next billing date. In the case of a declined credit card, I understand that a 3% late fee will be applied to my account. I certify that I am an authorized user of this credit card and will not dispute these scheduled transactions with my credit card company; so long as the transactions correspond to the terms indicated in this authorization form.
